Acknowledgment Checklist
Franchisee attestation must confirm:
- Received complete FDD including all exhibits, FPRs (if any), and state addenda on [spelled-out date]
- Understands 14-calendar-day waiting period before signing or paying (except permitted escrow under § 436)
- Afforded opportunity to consult legal counsel, accountants, and advisors
- No oral or written representations contradict or supplement the FDD
- If Item 19 FPR exists: Received and reviewed FPRs; understands their limitations
- If no Item 19 FPR: Understands franchisor made no earnings claims; will not rely on unauthorized claims

Pitfalls and Checks
- Date format — always spell out the month ("January 15, 2025"); never numeric-only
- Entity signers — distinguish "individually" vs. "as [Title] of [Entity Name]"
- Registration states — add state-specific addenda acknowledgment for CA, NY, IL, MD, MN, VA, WA, WI; check uploaded documents for applicable states
- Exhibit completeness — cross-reference FDD table of contents to verify exhibit list is exhaustive
- Minimal placeholders — insert transaction data from uploaded documents; bracket only genuinely unknown fields
- State timing mandates — 16 C.F.R. § 436 governs federal requirements; always check for additional state-level timing or content rules
